Your initial consultation will consist of a thorough evaluation. We will review our findings with you and help determine the treatment option that would work best for you. In most cases treatment can be accomplished the same day as your consultation however, a complex medical history, need for sedation or complex treatment plan may require a second separate treatment visit on another day. If emergent treatment is needed, we will get you appointed right away.
Please assist us by providing the following information at the time of your consultation:
- Please provide us with your most recent dental insurance information
- Complete our online medical history form and bring a list of your current medications with dosages
- Inform us if your physician requires you to pre-mediate with antibiotics prior to your dental visits due to joint replacement or cardiac concerns.
- If you are interested in any sedation options (nitrous oxide gas, Oral sedation (valium), or IV Sedation (going to sleep for your procedure) so that we can plan accordingly
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
